THE VILLAGE OF HEALING CENTER FAMILY NURSE PRACTITIONER (FNP-BC)

Village of Healing is a 501c3 community development organization with a new approach to community wellness. By focusing on healing and empowering individuals in the village, we work to eliminate social determinants of health and decrease health disparities through implementing community programming, advocacy, and collaboration. Our vision is to offer all members of the community life-empowering skills through multiple sources and mediums that will in turn heal and empower the village. We also work to ensure our constituents are equipped to lead healthy lives that thrive beyond survival.

POSITION SUMMARY:
We are seeking a mission-drive Family Nurse Practitioner (FNP) dedicated to advancing high-quality, family-centered, and culturally sensitive care for Black families across the lifespan.

This role focuses on the care of men, women, children with an emphasis on whole-listic care, health equity, prevention, and long-term family health outcomes.

The ideal candidate practices with clinical excellence, compassion, and accountability, using evidence-based healthcare and value-based care principles to improve patient outcomes, enhance patient satisfaction, and support the growth and sustainability of the practice.

  • SUMMARY OF RESPONSIBILITIES:
  • Patient Care & Clinical Practice
  • Provide comprehensive, patient-focused primary care to men, women, and children, addressing acute, chronic and preventative care needs
  • Deliver family-centered and holistic care addressing medical, behavioral, social, and cultural determinants of health
  • Utilize evidence-based clinical guidelines for assessment, diagnosis, treatment, and follow-up
  • Promote preventive care, chronic disease management, and patient education through shared decision-making
  • Provide health screenings, wellness visits, immunizations, and risk reduction counseling

Value-Based Care, Quality & Outcomes

  • Actively contribute to value-based care performance measures and AAAHC-aligned quality metrics,
  • including preventive screenings, chronic disease outcomes, patient satisfaction, and care continuity

Safety, Infection Prevention & Compliance

  • Follow AAAHC-compliant infection prevention and control standards, including hand hygiene,

  • Maintain accurate and compliant clinical documentation

Collaboration & Community Engagement

  • Collaborate with interdisciplinary care teams and leadership to deliver coordinated care

Practice Sustainability & Billing Knowledge

  • Demonstrate knowledge of effective documentation, coding, and billing practices

REQUIRED QUALIFICATIONS:

  • Active Family Nurse Practitioner (FNP) license
  • National certification (AANP or ANCC)
  • DEA license or ability to obtain
  • Experience in primary care, family medicine, pediatrics, or community health
  • Commitment to culturally responsive and family-centered care

PREFERRED QUALIFICATIONS:

  • Experience providing care for underserved populations
  • Strong interest in community-based care, older adult care, men’s health, and advancing health equity
  • Knowledge of quality measures, value-based care models, and population health approaches

SALARY:
$105,000 – $120,000 per year (Commensurate with experience)
